Public Benefits & Government Programs
We assist families in applying for and maintaining essential public benefits. This includes guidance through Medicaid applications (including waiver and long-term care programs), applying for services through the Division of Developmental Disabilities (DDD), and securing Social Security benefits such as SSI and SSD.
We also help families explore and apply for additional supports, including SNAP (food assistance), Energy Assistance, and TANF when appropriate. If applications are delayed or denied, we help you understand your options and navigate next steps.
Housing, Transportation & Community Living
Stable housing and reliable transportation are essential to independence. We assist families in exploring appropriate housing options, whether independent living, supported living, or more structured settings. We also help connect individuals to specialized transportation services and community-based supports that promote inclusion and safety.
Healthcare Coordination & Home Supports
We coordinate medical and behavioral healthcare services to ensure continuity and follow-through. This may include scheduling and attending medical appointments, facilitating communication between providers, arranging neuropsychological or psychiatric evaluations, and helping families understand treatment recommendations.
We also provide home health referrals and connect individuals with qualified direct support professionals and care agencies. Our role is to make sure services are not only in place, but functioning effectively.
Mental Health, Advocacy & System Navigation
Navigating healthcare and disability systems can be overwhelming. We advocate for individuals when services are delayed, insufficient, or improperly implemented. We help families understand their rights, prepare for meetings, and communicate effectively with agencies and providers.
We connect clients with therapists, psychologists, psychiatrists, and other specialists when mental health support is needed. Our focus is always on protecting dignity while ensuring access to appropriate care.
Vocational & Life Skills Support
Meaningful activity and employment are important parts of adult life. We assist with vocational referrals, day programs, and skill-building opportunities that support long-term independence and community engagement.
