Spectrum Care Management and Counseling provides support coordination to adults with developmental disabilities across Camden County. Our coordinators know the DDD system because they work in it every day. We have been doing this work since 2012, and we write and approve support plans in-house, which means faster turnaround for the families we serve.
What our Camden County coordinators do
- Develop the Individualized Service Plan around your family member’s goals
- Connect you with DDD-funded services and providers that fit
- Help you understand and use the DDD budget
- Monitor services and make changes when something is not working
- Stand with you at ISP meetings and whenever big decisions come up
Who we serve
Spectrum serves adults 21 and older who are eligible for DDD’s Supports Program or Community Care Program and have Medicaid. Not yet enrolled with DDD? Start your application at nj.gov/humanservices/ddd.
You have the right to choose your support coordination agency, and you can change agencies at any time if the fit is not right.
